Senior Care Manager - Adult Intensive Services

Deliver care management services administratively and clinically to an assigned caseload of clients with mental illness, substance use disorders and/or co‑occurring disorders. Provide mentorship and training to new staff and collaborate with leadership on special projects.

• Provide direct services and support to clients
• Coordinate care
• Mentor and train Care Managers
• Assist with onboarding requirements
• Complete projects to support program development, data collection and analysis

• Three years of experience working in Human Services, particularly in mental health, substance use, or developmental disability field
• Two years of experience as a Care Manager or comparable role
• Valid driver’s license and use of a personal vehicle

• Full Time

• Starting at $24.34 an hour; competitive salary commensurate to experience
