Outreach Clinical Coordinator

Provides community-based, low-barrier support to individuals experiencing mental health and/or substance use challenges and/or unmet social service needs.

Essential Functions:

Conduct initial screenings with community members to determine appropriate support, services, or referrals. Build and maintain relationships with community members, law enforcement, emergency medical services, providers and community partners. Provide referral services to members of the general public. This includes but is not limited to referring people for assistance with housing, addiction and mental health needs. Facilitate meetings between members of the public in need and community organizations that can provide assistance.

These organizations include the local community mental health agency, the local recovery centers, the local anti-poverty agency, the school department, other city departments, and any other agencies that can assist with the goal of helping people in need In coordination with the South Burlington Police, and on an ongoing basis, review police and fire activity logs including but not limited to domestic disturbances, drug overdoses, suicide attempts, death investigations, etc.

In coordination with the City of South Burlington coordinate referrals for South Burlington residents in need of services that wish not to contact emergency services. Assist individuals who experience barriers to accessing routine care and social services. Support individuals in developing strategies to reduce repeated use of police, emergency services, and hospital emergency departments. Collaborate, consult, and coordinate with service providers across the system of care.

Complete all required documentation, including statistical, clinical, and outcome reports. Perform related duties as assigned, within scope of practice. Requirements:
Master's degree Four years of relevant experience Valid driver's license and access to a personal vehicle. Knowledge of community resources. Strong engagement, de-escalation, and conflict resolution skills. Ability to problem-solve and navigate complex systems of care. Working knowledge of the Vermont System of Care and emergency services. Structure:
