RN Case Manager

This is a full-time, on-site RN Case Manager role based in Burlington, WI. The RN Case Manager will oversee comprehensive case management for hospice patients, including coordination of care, education, and support for patients and their families. Responsibilities include assessing patient needs, plan development, collaboration with interdisciplinary teams, and monitoring care plans to ensure optimum comfort and dignity. You will also manage discharge planning and effective use of healthcare resources.

• Experience in Nursing and Medicine with a focus on patient care and clinical expertise
• Proficiency in Case Management and Discharge Planning to coordinate and oversee care plans effectively
• Knowledge of Utilization Management to ensure appropriate use of healthcare resources
• Excellent communication and interpersonal skills for working with patients, families, and healthcare teams
• Ability to work on-site full-time and provide compassionate, patient-centered care
• Current RN license in the state of Wisconsin; previous hospice care experience is preferred
• CPR certification and familiarity with regulatory compliance standards are a plus
