Job SummaryWe are seeking a compassionate and experienced Registered Nurse (RN) to provide high-quality hospice care to patients in a home setting. This role focuses on delivering patient-centered care that prioritizes comfort, dignity, and emotional support for individuals facing life-limiting illnesses. The ideal candidate will be skilled in hospice practices and committed to supporting both patients and their families through every stage of care.
Key Responsibilities- Deliver comprehensive nursing care to hospice patients in accordance with established care plans.
- Assess patient conditions, monitor symptoms, and implement appropriate interventions to ensure comfort and quality of life.
- Collaborate with interdisciplinary teams including physicians, social workers, and therapists to coordinate care.
- Educate patients and family members on disease progression, medications, and end-of-life care strategies.
- Maintain accurate and timely clinical documentation.
- Provide emotional support and compassionate guidance to patients and caregivers.
- Ensure compliance with clinical standards, policies, and regulatory requirements.
- Active Registered Nurse (RN) license.
- Must have prior hospice experience.
- Strong clinical assessment and critical-thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently while managing patient needs effectively.
- No on-call requirements
- No holiday coverage
- No floating requirements
