RN Case Manager – Home Health Position Overview

We are seeking an experienced Registered Nurse (RN) Case Manager to join a compassionate Home Health / Hospice team serving patients in the Burlington and Walworth community
. The RN Case Manager will be responsible for coordinating and managing care for patients with end-stage illnesses across various home-based settings.

This role involves providing high-quality patient care, collaborating with interdisciplinary teams, and ensuring comfort-focused care for hospice patients.

• Average Daily Census: 105 patients
• RN Case Managers: 7
• Caseload: 14–16 patients per RN
• CNA Support: 6
• Minimum Experience
  
  Required:
  
  2 years
• First-Time Travelers: Experience preferred

The RN Case Manager will care for end-stage hospice patients across multiple care environments including:

• Private Homes
• Assisted Living Facilities (ALF)
• Skilled Nursing Facilities (SNF)
• Group Homes
• Independent Living Communities
• Two Hospitals for General Inpatient (GIP) Hospice Care

• Active Registered Nurse (RN) License
• BLS (Basic Life Support) Certification
• Valid Driver’s License
• Auto Insurance
• Electronic Medical Record (EMR): Epic and Rover
• IV Pumps: Available
• Medication Dispensing Systems
• Patient Monitoring: Chair and bed alarms
• Supplies: Home health bag and trunk supplies provided

• Required within the community to patient homes

• 1 week unit orientation

• Shift: 1st Shift
• Hours: 8:00 AM – 4:30 PM
• Days: Monday – Friday
