Position Details
- Department: 34141 WI Hospice Divisional – Nursing: SCT (Supportive Care Team)

This is a Per Diem position with flexibility to pick up shifts. The requirement is 2 shifts a month, including at least 1 weekend shift and 1 holiday shift per year.
Pay Range$35.50 - $53.25 per shift
Major Responsibilities- Nurse/Patient Relationship
- Identify the need for various communication styles and seek assistance in utilizing the most appropriate strategy.
- Identify factors affecting patient’s physical and psychosocial comfort and attend to these needs per standards.
- Identify the need for patient and family participation in care and seek assistance to assess readiness.
- Identify the need for an individualized, holistic approach to care and seek resources to meet those needs.
- Assuring/Improving Quality Care
- Identify patient and family concerns and issues and seek assistance as needed to advocate for them.
- Identify current literature in the area of practice.
- Incorporate nursing standards of care into practice.
- Identify and report potential or actual safety issues.
- Meet regulatory and mandatory requirements on the unit.
- Patient/Staff Education
- Identify patient and family learning needs and initiate and evaluate teaching using protocols and care standards.
- Recognize own abilities and contribute to a learning environment.
- Nursing Process
- Complete assessment and identify appropriate patient problems.
- Develop, implement, and evaluate plan of care on assigned patients.
- Prioritize and organize patient care and ensure follow‑through with the plan.
- Demonstrate skill and judgment necessary to implement interventions and procedures safely.
- Apply ethical decision‑making.
- Monitor confidentiality and assure appropriate business conduct.
- Delegate correctly to unlicensed assistive personnel.
- Seek resources for disruptive behavior that impedes care delivery.
- Leadership/Collaboration/Professional Development
- Contribute to effective operation of the unit.
- Identify effective communication and feedback skills and seek assistance as appropriate.
- Set professional practice goals with guidance and explore opportunities for growth.
- Earn a minimum of 5 contact hours per year.
- Registered Nurse (RN)
- Current CPR certification
- Valid driver's license issued by the Division of Motor Vehicles
- Graduate from an accredited school of nursing
- Earn 5.0 continuing education credits annually
- 1 year of medical nursing experience with various age groups
- Good communication and interpersonal skills
- Good documentation skills
- Computer skills
- Good time management and organizational skills
- Familiarity with patient care equipment
- Ability to work weekends, holidays, and different shifts to accommodate staffing.
- Ability to take on‑call assignments.
- Ability to handle multiple tasks and complete work within short time frames.
- Ability to transport objects and equipment.
- Ability to respond quickly to patient needs and work at a fast pace.
- Work environment may expose to adverse, hazardous, or unpleasant conditions.
- Routinely work with potentially infectious specimens and patients.
- Ability to lift up to 100 pounds occasionally, 50 pounds frequently, and 20 pounds constantly.
- Must be able to travel to all areas within the Advocate service area in all extremes of weather.
- Must be willing to work occasional overtime if needed.
- Must be able to assist with ambulation, including turning and transferring patients up to 200 pounds.
- For patient lifts over 35 pounds, use patient handling equipment and may need another associate.
