Licensed Social Worker- SNF Social Services Director

We're seeking a strong candidate to fill a Social Services Director position – Social Worker with experience in long‑term care preferred to support the social and emotional well‑being of our residents.

• Support residents in maintaining or improving physical, emotional, and psychosocial well‑being
• Identify and assess individual social and emotional needs
• Create and update personalized care plans
• Participate in admission planning, discharge coordination, and transfers
• Collaborate in interdisciplinary care plan meetings
• Partner with finance to ensure residents understand documentation and financial needs
• Coordinate discharge planning, including DME and home care services for safe community reintegration

• §483.70(o)(1) An individual with a minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology; and
• §483.70(o)(2) One year of supervised social work experience in a health care setting working directly with individuals
• Experience working with aging and diverse populations
