POSITION SUMMARY:
The Visiting Nurse RN is responsible for assessments, skilled treatments, teaching patients/caregivers in the home, coordinating with other disciplines and community agencies, and accurate and timely documentation. Must be capable of autonomy of action, judgement and unbiased attitude and flexibility.
This job description is intended to identify some of the primary duties and responsibilities. Mayers Memorial Hospital District reserves the right to modify, supplement, delete or augment the duties and responsibilities specified in this position description, at MAYERS MEMORIAL HOSPITAL DISTRICT'S sole and absolute discretion.
Position Qualifications
Current California R.N. LicenseMinimum of one (1) year acute care experience within the past three (3) years preferredMaintains current ACLS certification.Position Responsibilities
Able to process complete referrals for prospective patients in a timely manner.Able to travel to multiple patients per day.Perform the initial evaluation visit to determine home care needs in accordance with provider's orders, policies, and procedures. Develop, implement, evaluate and update patient's plan of care on an ongoing basis. Monitor homebound status. Refer to community resources when necessary to meet patient's goals.Provide skilled nursing care and assessments to patient in the home environment, according to the plan of care, and make necessary revisions as patient status and needs change; identify recertification and discharge planning needs; utilize appropriate assessment and documentation. Include the patient and the family/caregivers in the planning process. Patient records will reflect the needs and reachable goals within a specific time frame.Initiate appropriate preventive nursing procedures; administer and educate on medications, wound care/dressing changes, and treatments as prescribed by the provider; purge all expired medications. Monitor vital signs, blood pressure, heart rate, temperature, and oximeter statistics; monitor blood glucose levels; provide clear and precise teaching and support to patient and family/caregivers.Document all care, intervention and care coordination. Information will be documented accurately, completely, and in a timely manner. Documentation will reflect delivery of quality and safe home care services.Update patient's provider when necessary and at least every sixty days. Receive provider orders accurately as required. Effectively communicate with referral sources and other team members regarding patient needs and report any changes in the patient's condition in a timely manner.Attend and participate in patient case conferences to demonstrate coordination and continuity of patient care.Implement Clinic's infection control, safety measures, and universal precautions.Knowledgeable and able to perform in the organization's computer systems.Able to perform CLIA waived lab tests and communicates results to provider.Performs other duties as required.Professional Requirements
Adheres to dress code; appearance is neat and professional.Completes annual education requirements if applicable.Maintains regulatory requirements.Wears identification while on duty.Attends annual evaluation and participates actively in this process.Reports to work on time and as scheduled; completes work in designated time.Attends all meetings as appropriate.Exhibits the mission, ethics and goals of Mayers Memorial Hospital District in the performance of job duties.Working Conditions
