Adult Day Program Assistant – Burnsville, MN

Ebenezer is looking for an Adult Day Program Assistant to join our team in Burnsville, MN. This role encourages socialization and assists in planning and implementing active programming for individual senior day center participants and child care groups. The assistant is responsible for ongoing charting, case management, and assisting with personal care duties.

Ebenezer Ridges Care Center offers a full continuum of care campus, including Transitional Care, Adult Day Programs, Intergenerational Child Care, Long Term Care, Enhanced Care, and Assisted Living/Memory Care. We are located across the street from Fairview Ridges Hospital at the I-35E and I-35W split in Burnsville, MN. • Provide direct care for Adult Day Program participants.
• Prepare, implement, and clean up daily programs (e.g., crafts, woodworking, games, music).
• Perform routine personal care and grooming, including baths.
• Assist participants with toileting needs.
• Communicate participants’ needs and individual care plans to staff.
• Report problems, progress, and observations to the Director.
• Assist the Director of Adult Day with individual case management.
• Maintain monthly charting, quarterly care plans, and health updates.
• Maintain a clean and safe environment for participants, including safe areas and adequate supplies.

• Current driver’s license in good standing.
• 0 to 1 year of previous experience.

• CNA experience (or direct care experience in lieu of certification).
• Experience with Windows/Microsoft applications.
• Customer service skills.
• Empathetic and effective communication with tenants/clients, families, etc.
• Interpersonal skills; assist with ADL’s.
