We are seeking a dedicated and compassionate Home Health Registered Nurse (RN) to join our team in Burnsville. As a Home Health RN, you will play a critical role in providing high-quality, patient-centered care to individuals in their homes, promoting their independence and well-being.

• Conduct comprehensive assessments of patients' health status and develop individualized care plans.
• Administer medications, treatments, and procedures as prescribed by the physician.
• Monitor patients' progress and adjust care plans as necessary to ensure optimal health outcomes.
• Educate patients and their families on managing illnesses or injuries and promote healthy lifestyle choices.
• Collaborate with other healthcare professionals to coordinate patient care and ensure continuity of services.
• Maintain accurate and timely documentation of patient care activities and progress notes.
• Ensure compliance with all state and federal regulations and standards of care.

• Current and unrestricted RN license in the state of Minnesota.
• Bachelor's degree in Nursing (BSN) preferred.
• Minimum of one year of nursing experience.
• Require Home Health Experience.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Reliable transportation and valid driver's license.
