Job Title: Registered Nurse (RN) Case Manager – Home Health | Build Your Day, 1:1 Patient Care
Location: Greater Twin Cities Metro Area (21-county service area)
Schedule: Full-Time, Monday to Friday, 8:00 AM – 5:00 PM, 1 weekend per month, 1 on-call shift per week
Compensation: $85,000 – $130,000 annually plus $6,000 in first-year bonuses

- Manage a home health caseload across homes and community settings.
- Complete OASIS assessments and individualized care plans
. - Provide skilled nursing care, education, and coordination
. - Collaborate with physicians, therapists, and interdisciplinary teams.
- Support patients in maintaining independence and safety at home
. - Maintain accurate, timely documentation
.
- 3+ years of RN experience, home health preferred.
- Strong clinical judgment and independence.
- Organized, self-directed, and comfortable in a field role.
