Salary Range: $26.50 To $26.50 Hourly

Living Well Disability Services delivers exceptional services that transform the lives of people impacted by disabilities. With 36 group homes across the 10‑county area we specialize in a holistic approach to wellness and well‑being for residents.

As a Staff Nurse you will provide direct nursing support and oversight of medical care for residents. The role involves pairing with home managers and staff to ensure cohesive medical support according to each resident’s needs. You will supervise unlicensed staff regarding health‑related issues, and work directly with residents to provide optimal health care. Caseload of approximately 1‑4 homes.

• Planning:
  Follow medication and health care policies, evaluate residents’ status, develop health care profiles and nursing assessments.
• Organization:
  Train and mentor staff to support residents, ensuring understanding of health plans and providing additional training.
• Communication:
  Cooperate with providers, interdisciplinary teams, and organizational staff; answer health and medication questions.
• Teamwork:
  Support residents and team members to maintain health, access communities, and meet outcomes.

• Current LPN license.
• Valid MN Driver’s License.
• Pass a MN DHS Background Study.
