Job Title:
Registered Nurse (RN) Case Manager – Home Health | Your Patients Deserve More Than a Rushed Visit
Territory: Burnsville, Eagan + 35 mile radius from home
Schedule: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM | 1 Weekend Per Month | 1 On-Call Shift Per Week
Compensation: $85,000 – $130,000 annually plus $6,000 in first-year bonuses

- Manage a home health caseload throughout Woodbury, Cottage Grove, Hastings, St. Paul Park, and Landfall.
- Complete comprehensive OASIS assessments and individualized care plans.
- Deliver skilled nursing care and patient education.
- Collaborate closely with physicians and interdisciplinary team members.
- Help patients recover, remain independent, and avoid unnecessary hospitalizations.
- Maintain accurate clinical documentation.
- Active Minnesota Registered Nurse (RN) license.
- 3+ years of RN experience (home health experience preferred).
- Strong assessment and critical thinking skills.
- Excellent communication and organizational abilities.
- Reliable transportation for local travel.
