Gateway Unlimited Living LLC is a Minnesota-licensed 245D provider committed to helping individuals live more independently, participate in their communities, and achieve meaningful personal and employment goals.
Position SummaryThe In-Home Support & Employment Specialist provides individualized, person-centered services in individuals' homes and throughout their communities.
The position will provide Individual Home Support with Training as well as Employment Services, depending on the needs and goals of the people served.
The successful candidate will help individuals develop and maintain skills that promote independence while also supporting individuals in exploring, obtaining, and maintaining meaningful employment.
Key Responsibilities In-Home Support with Training- Provide individualized, person-centered support in the individual's home and community.
- Teach and reinforce independent living skills.
- Assist teaching individuals with developing skills related:
- Household responsibilities
- Meal planning and preparation
- Organization
- Personal safety
- Community participation
- Communication and social skills
- Money management and budgeting
- Transportation and navigating the community
- Health and wellness routines
- Support individuals in achieving goals identified in their person-centered and service plans.
- Encourage independence rather than doing tasks for the individual.
- Provide positive reinforcement and skill-building opportunities.
- Assist individuals in developing confidence and self-advocacy skills.
- Help individuals identify employment interests, strengths, abilities, and goals.
- Assist with job exploration and career development.
- Support individuals with job applications, resumes, interviews, and workplace preparation.
- Develop relationships with employers and identify potential employment opportunities.
- Provide on-the-job training and coaching when appropriate.
- Teach workplace expectations, communication, and job-related skills.
- Support individuals in maintaining successful employment.
- Coordinate with individuals, families, employers, case managers, and other members of the support team.
- Document progress toward employment and individual goals.
- Daily/service documentation
- Progress notes
- Tracking progress toward individual goals
- Reporting significant changes or concerns
- Following individual service plans
- Maintaining confidentiality
- Following 245D policies and procedures
- Participating in required staff meetings and trainings
- Maintaining required training and qualifications

- Comfortable providing services in individuals' homes and communities

- Comfortable working with individuals with disabilities

- Flexible and willing to travel between service locations
- Able to maintain professional boundaries and confidentiality
- Previous experience in disability services, direct support, employment services, behavioral support, human services, or community-based services is preferred but not required.
- High school diploma or equivalent preferred.
- Must meet applicable Minnesota 245D staff qualifications and training requirements.
- Must successfully complete required background studies.
- Must complete required 245D and company training.
- Valid driver's license and reliable transportation may be required based on assigned services.
- Ability to travel to individuals' homes, workplaces, and community locations.
- Ability to communicate effectively in written and spoken English.
- Ability to maintain accurate documentation.

Full-Time
Schedule may vary based on the needs, schedules, employment goals, and service plans of the individuals supported. Some flexibility in scheduling may be required.
Travel within the assigned service area is required.
