Wellness Nurse

Salary: $32.75 - $38.50 per hour, based on experience

Schedule: Full Time, Morning Shift, days vary; every other weekend commitment

We are seeking a Wellness Nurse to support the health and well‑being of residents through ongoing clinical monitoring and support. The role involves assisting community nurses, caregivers, and medication personnel to ensure regulatory compliance and clinical excellence. You must work effectively as part of a team, adapt to changing duties, and maintain a warm, encouraging disposition.

• Assist in coordination of the resident admission process including screening, assessment, and care needs
• Ensure care is delivered as indicated in resident care plans
• Medication and treatment administration and ensure medications are administered per physicians’ orders
• Assistance with coordinating care and services with other outside providers including resident physicians
• Document and report care provided and resident observations including unusual or significant changes
• Maintain a working knowledge of state regulations, policies, and procedures

• Working knowledge of state regulations, policies, and procedures, dictated for compliance
• Graduate from an accredited school of practical nursing
• Current nursing license and state registration (LPN or RN)
• Three years’ experience in assessment and geriatric nursing, preferred
• A big smile, eagerness to learn, and compassion for our Residents
