Position: REGISTERED NURSE (RN)

Position Overview

XCELHome Health is seeking a Registered Nurse for an exciting travel or local job in the locations identified below. The Registered Nurse coordinates all aspects of the client's home care and interacts with other disciplines as needed. She/he provides professional care to the client, as indicated by Agency policies, State and Federal laws/regulations. In‑home admission visits and eligibility determinations are part of the RN's duties, as is the development of the Care Plan.

The RN also prepares paperwork necessary for agency policies and federal/state laws and assists with the training and supervision of Care Aides.

Location(s): BURTONSVILLE / SPENCERVILLE / POTOMAC; MD

Reporting Relationship
  • Reports to Agency Manager/Administrator.
Qualification / Prerequisites
  • Graduate of an accredited school of professional nursing and currently licensed as an RN through the State Board of Nursing.
  • Minimum of one year of nursing experience, preferably with home care experience.
  • Good physical stamina and mental health with pre‑employment health clearance.
  • Licensed driver with at least a one‑year clean driving record.
  • Vehicle insured in accordance with state and/or Agency requirements and in good working order.
  • Excellent writing and charting skills.
  • Passes a background check and drug screen.
Responsibilities / Activities
  • Assess the home environment and determine the needs of clients.
  • Record the health needs of the client.
  • Initiate Nursing or Care Aide’s Care Plan and/or Homemaker/Companion Service Plan and associated paperwork in accordance with Agency policy.
  • Evaluate the effects of care and regularly re‑evaluate the client’s nursing needs.
  • Evaluate and revise the Care Plan as necessary.
  • Make referrals to other agencies or services as needed.
  • Coordinate the care of all assigned clients.
  • Prepare written instructions for the Care Aide.
  • Supervise the Care Aide in the home.
  • Instruct Care Aides in the proper use of practices, procedures, and equipment.
  • Participate in in‑service training programs and staff meetings.
  • Serve as liaison between clients and other health care professionals.
  • Teach, counsel, and demonstrate skills to clients and their families.
  • Administer medications, monitor clients and instruct on appropriate home care.
  • Provide services based on the physician’s orders such as urinary catheter insertion and care, venipuncture and administration of IV fluids or medications, and changing dressings.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Counsel the client and his/her family in meeting nursing and related needs.
  • Act as the coordinator of the health care team.
  • Provide reports to the client’s physician when the client’s condition changes or there are deviations from the Care Plan.
  • Prepare clinical and progress notes on relevant and specialized nursing services provided.
  • Maintain a clinical record for each client receiving care.
  • Submit documentation as per Agency policy.
  • Participate in clinical record reviews.
  • Direct activities of the Licensed Practical Nurse, if relevant.
  • Comply with Homecare Agency Compliance Plan, Infection Control Policies, HIPAA Policies, dress code, and all state and local regulations and accreditation standards.
  • Work weekends, holidays, and occasional overtime when requested.
  • Assume rotational on‑call responsibilities during non‑office hours.
  • Project a professional attitude toward self, clients, and other staff members.
  • Follow Agency public‑relation policies regarding outside speaking and professional affiliations, and clear related requests with Agency Manager/Administrator.
  • Assume other duties as requested by the Agency Manager/Administrator or Governing Board.
