Monday – Friday, 8:30 am – 5:00 pm

About the Role

The Enrollment Case Manager (ECM) supports the Pennsylvania Independent Enrollment Broker (IEB) program by conducting in-home assessments, processing applications, and facilitating eligibility determinations to help individuals and families access essential care services. This role combines field-based responsibilities with remote administrative tasks, contributing to timely and accurate program enrollment.

Responsibilities and Impact
  • Conduct 3-4 in-person intake visits daily in applicant's homes, nursing facilities, or shelters within assigned counties.
  • Process waiver program applications through COMPASS and coordinate with eligibility teams for accurate determinations.
  • Provide complete case management support from referral through program enrollment or closure.
  • Maintain accurate applicant records and data tracking using Microsoft Excel.
  • Offer clear, unbiased information regarding service options, rights, and program processes.
  • Travel within an assigned area using reliable, independent transportation.
  • Complete outreach or administrative work when not conducting field visits.
  • Attend required meetings, trainings, and team discussions.

These responsibilities are not exhaustive; additional tasks may be assigned.

Minimum Qualifications
  • Bachelor's degree with at least 12 credits in Sociology, Psychology, Social Welfare, or any Human Services or Social Science field.
  • Experience in case management, social services, or a related field.
  • Ability to work independently while managing multiple priorities and deadlines.
  • Strong organizational, documentation, and communication skills.
  • Proficiency with Microsoft Excel, Word, and Teams.
  • Reliable transportation and valid driver's license.
  • Ability to adapt quickly to changing processes and requirements.
  • Familiarity with COMPASS or similar eligibility systems
Standout Qualifications
  • Background in healthcare networks, disability services, or public benefit programs.
  • Bilingual in Spanish/English.
  • Strong cultural sensitivity and community awareness.

Pay Range: $20.50 - $21.50 with mileage and expenses fully reimbursed. After successfully completing a contingency period of at least 120 days with our company, eligible employees will have access to an excellent benefits package, including full insurance and other benefits effective the first of the month following the end of the probationary period. In addition, employees receive paid time off for Pennsylvania state holidays from day one, provided they work their scheduled hours before and after the holiday.
