Community Care Social Worker Case Manager

Position Overview

Benefis offers home and community-based case management services centered on health and human services. This role focuses on the human service aspect of case management for Montana Big Sky Waiver Medicaid beneficiaries who are elderly or physically disabled and reside in their own homes.

• Provide psychosocial assessments, mental health support, and navigate housing, financial stability, and community resources.
• Coordinate with the multidisciplinary Care Management Team to develop individualized member-driven planning, assessment, and monitoring of services.
• Deploy case management methods to meet the psychosocial needs of members and facilitate long term care placements.
• Handle pressure to meet deadlines, maintain accuracy, and adapt to constantly changing situations.
• Communicate with a variety of people, manage stressful situations, and resolve conflicts.
• Perform all assigned job duties and tasks.

• Bachelor’s degree in social work, psychology, sociology, or a related field.
• Three (3) years of social work experience in a health care setting (preferred).
• Knowledge of case management methods, practice, and procedures.
• Knowledge of diagnostic and crisis intervention skills.
• Knowledge of issues and needs of long term care consumers.
• Experience managing a budget.
• Understanding of human behavior, disabilities, and the aging process.
• Must live in or around Silver Bow County;
  Monday–Friday, no weekends or on‑call shifts.
• Compliance with all licensing board regulations and Benefis Health System policies.

Exempt or Non‑Exempt. Starting wage at $21.69 per hour, with actual offer based on applicable experience. Includes annual wage increase, health benefits, and a retirement plan with employer match.
