Concordia Visiting Nurses is seeking a dedicated and compassionate Mental Health Registered Nurse (MHRN), full-time, to join our team! Must have MH experience.

• Assessment and Planning:
  Conduct comprehensive assessments of patients' mental health status, develop individualized care plans, and collaborate with physicians and other healthcare providers.
• Skilled Nursing Care:
  Provide direct nursing care, including administering medications, monitoring vital signs, managing symptoms, and providing emotional support.
• Education and Support:
  Educate patients and families on medication management, coping strategies, and self-care techniques.
• Crisis Intervention:
  Provide crisis intervention and de-escalation techniques when necessary.
• Collaboration:
  
  Work collaboratively with other healthcare professionals, including physicians, therapists, and social workers.
• Documentation:
  Maintain accurate and detailed patient records.
• Case Management:
  Coordinate care and follow-up for patients, ensuring continuity of care.

• Assessing patients' mental health needs.
• Developing and implementing treatment plans.
• Administering medications and monitoring for side effects.
• Providing counseling and emotional support.
• Managing behavioral issues and crisis situations.
• Educating patients and families about mental health conditions.
• Collaborating with other healthcare professionals.
• Monitoring patient progress and making adjustments to the treatment plan as needed.
• Supervising and training home health aides.
• Discharging patients after consultation with the physician and Clinical Manager.
