Concordia Visiting Nurses is seeking a dedicated and compassionate Mental Health Registered Nurse (MHRN), full-time, to join our team! Must have MH experience.

- Assessment and Planning:
Conduct comprehensive assessments of patients' mental health status, develop individualized care plans, and collaborate with physicians and other healthcare providers. - Skilled Nursing Care:
Provide direct nursing care, including administering medications, monitoring vital signs, managing symptoms, and providing emotional support. - Education and Support:
Educate patients and families on medication management, coping strategies, and self-care techniques. - Crisis Intervention:
Provide crisis intervention and de-escalation techniques when necessary. - Collaboration:
Work collaboratively with other healthcare professionals, including physicians, therapists, and social workers. - Documentation:
Maintain accurate and detailed patient records. - Case Management:
Coordinate care and follow-up for patients, ensuring continuity of care.
- Assessing patients' mental health needs.
- Developing and implementing treatment plans.
- Administering medications and monitoring for side effects.
- Providing counseling and emotional support.
- Managing behavioral issues and crisis situations.
- Educating patients and families about mental health conditions.
- Collaborating with other healthcare professionals.
- Monitoring patient progress and making adjustments to the treatment plan as needed.
- Supervising and training home health aides.
- Discharging patients after consultation with the physician and Clinical Manager.
