Resident Care Coordinator; RN​/LPN – Lead Care & Family Liaison

Green Acres of Cadillac is seeking a Resident Care Coordinator to join our team. This is a rewarding opportunity for an LPN or RN, who is looking for a change of pace and a meaningful career serving seniors.

The ideal candidate is highly organized, an effective communicator, and has a genuine love for working with the elderly. This role plays a vital part in supporting residents, staff, and families while maintaining high-quality care standards within our community.

Key Responsibilities:

• Conduct and oversee
  level of care assessments
• Coordinate and monitor resident care needs
• Work closely with
  physicians, hospice providers, and families
• Support, educate, and collaborate with direct care staff
• Order and track
  medications/scripts
  as needed
• Enter and manage
  resident care orders
• Communicate effectively with residents, families, and leadership
• Assist with maintaining compliance with care standards, policies, and regulations
• Share
  on-call responsibilities
  as part of the leadership team
• Shares on-call responsibilities
• Wage based on experience and credentials

Qualifications:

• Current
  LPN or RN
  certification preferred
• Strong organizational and communication skills
• Leadership mindset with the ability to work collaboratively
• Compassionate, resident-centered approach to care
