Home Health RN Case Manager

Horizon Home Health & Hospice is seeking a full-time Registered Nurse Case Manager to join our home-health team in Caldwell, Idaho. This role is ideal for nurses who value autonomy, meaningful patient relationships, and the opportunity to deliver high-quality care directly in patients' homes.

As an RN Case Manager, you will oversee a caseload of home-health patients, provide skilled nursing interventions, coordinate interdisciplinary services, and support patients and families throughout their recovery or chronic-care journey.

Key Responsibilities

RN Responsibilities

  • Complete comprehensive nursing assessments and develop individualized care plans.
  • Provide skilled nursing interventions in patient homes.
  • Manage a balanced caseload with strong organizational efficiency.
  • Coordinate care with interdisciplinary team members to ensure seamless service delivery.
  • Educate patients and caregivers on disease processes, medications, and safety.
  • Maintain accurate, timely documentation in accordance with regulatory and organizational standards.
  • Communicate effectively with physicians, families, and internal team members.
Qualifications

RN Requirements

  • Current Idaho RN license (or ability to obtain).
  • One year of nursing experience preferred; home health experience a plus.
  • Strong assessment, communication, and critical-thinking skills.
  • Ability to work independently in a field-based role.
  • Reliable transportation and valid driver's license.
