Home Health RN Case Manager Horizon Home Health & Hospice is seeking a full‑time Registered Nurse Case Manager to join our home‑health team in Caldwell, Idaho. This role is ideal for nurses who value autonomy, meaningful patient relationships, and the opportunity to deliver high‑quality care directly in patients’ homes. As an RN Case Manager, you will oversee a caseload of home‑health patients, provide skilled nursing interventions, coordinate interdisciplinary services, and support patients and families throughout their recovery or chronic‑care journey.

- Complete comprehensive nursing assessments and develop individualized care plans.
- Provide skilled nursing interventions in patient homes.
- Manage a balanced caseload with strong organizational efficiency.
- Coordinate care with interdisciplinary team members to ensure seamless service delivery.
- Educate patients and caregivers on disease processes, medications, and safety.
- Maintain accurate, timely documentation in accordance with regulatory and organizational standards.
- Communicate effectively with physicians, families, and internal team members.
- Current Idaho RN license (or ability to obtain).
- One year of nursing experience preferred; home health experience a plus.
- Strong assessment, communication, and critical‑thinking skills.
- Ability to work independently in a field‑based role.
- Reliable transportation and valid driver’s license.
