Prospect, a leading not-for-profit in Alberta, is searching for a Service Coordinator. This role is responsible for guiding clients from intake to service completion while ensuring communication and support throughout the process. You will coordinate service plans, develop assessments, and engage with support networks for optimal client outcomes.
Key Responsibilities:
• Serve as the central contact for client services
• Conduct client assessments and consultations
• Schedule and facilitate various client meetings
• Develop and monitor service plans and client goals
• Maintain accurate client records and reporting
Requirements:
• Post-secondary education in related fields
• Minimum 2 years in case management or social services
• Proficient in Microsoft Office and database software
• Strong communication and organizational skills
• Empathetic, driven, and adaptable under pressure
