Monday - Friday 8:30am - 5pm*
* + Directs daily operations of the care management department, including discharge planning, care coordination, progression of care, and social services while ensuring compliance with CMS CoPs and federal/state regulations.

+ Oversees recruitment, retention, staffing, and leadership development activities to ensure strong care management team performance.

+ Collaborates with interdisciplinary hospital leaders, executives, post-acute care entities, and Utilization Management to improve patient outcomes, care transitions, and reduce readmissions.

+ Identifies and leads operational, workflow, and technology improvement initiatives to enhance staff efficiency, patient outcomes, and departmental performance.

+ - Develops, implements, and ensures compliance with care management policies, standard operating procedures, operational strategies, and fiscal accountability measures.

** Knowledge, Skills, and Abilities:*
* + Knowledge of change management principles, methodologies, and tools [Required]

* + Master's in Social Work  
** OR
* * Bachelor's in Social Work & Master's in related field

* Work Experience:

* + 5+ direct care management experience [Required]
