Job Description:

Minimum Pay: $35 / hour

Work Schedule: 3 x 12 hour shifts - 7pm-7am, rotating weekends

Sign on Bonus: $20,000 for eligible candidates

Responsibilities
  • Conduct comprehensive patient assessments through observation, interviews, and physical examinations to gather patient history and vital signs, identifying physical, psychosocial, cultural, and spiritual needs.
  • Develop and implement individualized care plans based on assessment data, setting SMART goals in collaboration with patients, families, and healthcare teams.
  • Administer medications and treatments safely, including oral, intravenous, and intramuscular forms, and perform procedures such as wound care, catheterization, and IV therapy.
  • Provide patient and family education about health conditions, treatments, and self-care practices, and develop discharge instructions and follow-up care plans.
  • Advocate for patient health and safety by acting as a liaison between patients and the healthcare system, protecting patient rights and confidentiality, and securing necessary resources to improve patient outcomes.
  • Participate in performance improvement activities by engaging in unit-based quality improvement, evidence-based practice, and research initiatives, applying current healthcare research findings to expand clinical knowledge and using quality measures to enhance outcomes.
  • Collaborate with healthcare professionals to ensure coordinated and comprehensive patient care, maintaining open and clear communication with all team members.
  • Collect and analyze patient health data from patients, families, and healthcare providers and prioritize patient needs based on their condition to ensure timely and effective care.
  • Document patient care activities accurately in medical records, including assessments, care plans, interventions, and patient responses to ensure continuity and quality of care.
  • Evaluate and adjust care plans by monitoring effectiveness and making necessary adjustments based on ongoing assessments and feedback.
  • Perform nursing procedures such as wound care, catheterization, and specimen collection with accurate and safe execution.
  • Ensure patient safety by adhering to policies and procedures, maintaining a safe environment, and participating in bedside shift reports for continuity of care.
  • Performs other duties as assigned.
Knowledge, Skills, and Abilities
  • Administers all types of medications correctly per age group for all populations served according to policy. [Required]
  • Demonstrates proficient technical skills and operational knowledge of all equipment used on the unit. [Required]
  • Performs daily rotational task assignment as directed. [Required]
  • Supervises Nursing Assistants in basic nursing functions and ensures safe patient ambulation and transfers, turning, repositioning, and care for bedfast or incontinent patients every two hours. [Required]
  • Serves as a preceptor as necessary. [Required]
  • Communication/Collaboration. [Required]
  • Gather data/assess/reassess all populations served and perform primary assessment of all newly admitted or transferred patients and complete care plans. [Required]
  • Documents/communicates clearly patient data, initiates and implements orders promptly, and collaborates with the healthcare team under HIPAA. [Required]
  • Collaborates on Care Plan input. [Required]
  • Timely collaboration to establish priorities in planning, organizing, and implementing patient care based on nursing assessment and patient/family needs. [Required]
