Location: Willows
Pneuma Care Health and Wellness, under MACC, seeks a Community Care Coordinator to implement Enhanced Care Management (ECM) and Community Supports. You will coordinate plans, engage members in their homes or via phone, arrange transportation, and collaborate with providers to ensure seamless service delivery.
Qualifications include bilingual English/Spanish and 2+ years in case management or related social services, with AA in a relevant field preferred. Travel to sites may be required.
