This individual will be responsible for assisting in developing nutrition workflows, protocols, screening tools, referral criteria, and documentation standards to support patient and community wellbeing. The ideal candidate is bilingual in English and Spanish, compassionate, innovative, and connective to diverse groups of people.

Hours per Week: 32 hours/week (28 clinical/programmatic, 4 administrative) = full-time at full salary. Will require 1-2 evening shifts per week.

A Day in the Life of a Registered Dietitian Nutritionist with Winters Healthcare
If you were a Registered Dietitian Nutritionist with Winters Healthcare, you would spend your day embedded within our integrated primary care teams. Patients may be introduced to you through a warm handoff from a physician, physician assistant, nurse practitioner, behavioral health clinician, dentist, or population health coach. You might meet with a newly diagnosed patient with diabetes, help a pregnant patient navigate healthy eating during pregnancy, provide medical nutrition therapy for a patient with obesity and hypertension, or assist a family experiencing food insecurity.
You will provide both individual and group services. Individual visits may include comprehensive nutrition assessments, care plan development, chronic disease management, and follow-up counseling. Group services include a 10-week Nutrition and Lifestyle Medicine Program featuring nutrition education, cooking demonstrations, physical activity, stress management, goal setting, and peer support. The RDN will oversee group programming and support curriculum development and quality improvement activities.
Beyond direct patient care, you will work closely with medical providers, behavioral health clinicians, dental teams, population health staff, and community partners to improve access to nutritious foods. The ideal candidate enjoys building relationships, creating innovative programs, educating patients and staff, and helping individuals make sustainable lifestyle changes that improve long-term health outcomes.

Summary of Duties:
Under the supervision of the Director of Clinical Services, the Registered Dietitian (RD) serves as the clinical nutrition lead for individual and group-based nutrition services. This position provides medical nutrition therapy, nutrition education, and counseling services while partnering with interdisciplinary care teams to prevent and manage chronic conditions, address food insecurity, and improve health outcomes through integrated, population health-focused approaches.
Responsibilities
- Provide individualized medical nutrition therapy for pediatric, adult, prenatal, and geriatric patients.
- Conduct comprehensive nutrition assessments and develop individualized care plans.
- Manage nutrition care for patients with diabetes, prediabetes, hypertension, obesity, underweight, cardiovascular disease, pregnancy-related nutrition needs, and food insecurity.
- Document care in Epic EHR and participate in team-based care planning.
- Use evidence-based interventions consistent with current nutrition practice standards.
Program Development and Leadership
- Assist in developing nutrition workflows, protocols, screening tools, referral criteria, and documentation standards.
- Design and maintain patient education materials, care pathways, and community resource directories.
- Assist in developing and overseeing nutrition-related clinical templates, assessment tools, and care plans.
- Serve as the organization's nutrition subject matter expert.
- Attend at least one nutrition-focused state or national conference per year.
Group Visits and Lifestyle Medicine
- Lead or oversee nutrition and lifestyle medicine group visits.
- Collaborate with the Population Health Coach to deliver a 10-week Nutrition and Lifestyle Medicine curriculum.
