** to join rehabilitation team. Opportunity to work in collaborative skilled nursing facility environment focused on delivering high-quality patient-centered care.### Position Summary Under supervision of licensed Physical Therapist, PTA provides direct resident care and assists in implementing individualized treatment plans to improve mobility, function, and overall quality of life.###

Key Responsibilities
* Provide treatment to residents as directed by Physical Therapist
* Assist in implementing individualized therapy care plans
* Complete timely, accurate, and compliant documentation per regulatory and facility requirements
* Maintain safe treatment environment, adhering to infection control and safety protocols
* Communicate effectively with interdisciplinary team, including therapists, nursing staff, physicians, and families regarding patient progress
* Participate in care plan meetings and rehabilitation conferences as needed
* Educate residents and families on therapy goals, exercises, safety, and discharge planning.
* Assist with training restorative aides and supporting facility therapy programs### Qualifications
* Current California Physical Therapist Assistant (PTA) license required
* Graduate of accredited PTA program
* Previous experience in skilled nursing facility (SNF) preferred
* Strong communication and teamwork skills
* Ability to maintain compliance with all state and federal regulations###

Physical Requirements
* Ability to lift, transfer, and assist residents safely
