About the Position
Registered Nurse (RN) Case Manager is responsible for coordinating and managing patient care across the continuum, ensuring safe, efficient, and cost-effective outcomes. Acts as a liaison between patients, families, physicians, and interdisciplinary care teams to develop, implement, and evaluate individualized care plans. Ensures compliance with regulatory standards and hospital policies while optimizing patient outcomes.

Qualifications:
Active RN License in the state of practice.
BLS Certification required; ACLS may be preferred depending on facility.
Minimum of 2–3 years clinical nursing experience , preferably in acute care or relevant specialty.
Strong communication, organizational, and critical thinking skills.
Experience with case management, utilization review, or discharge planning is preferred.
Knowledge of healthcare regulations, insurance requirements, and community resources.
