Position Overview:

The Case Manager Registered Nurse (RN) is responsible for coordinating and managing patient care across the continuum to ensure high-quality, cost-effective outcomes. This role involves collaboration with interdisciplinary teams, patients, and families to facilitate efficient transitions of care and promote optimal recovery.

Key Responsibilities:

Assess, plan, implement, and evaluate individualized care plans for assigned patients.

Coordinate patient discharge planning and post-acute care arrangements.

Work collaboratively with physicians, nurses, and other healthcare professionals to ensure continuity of care.

Monitor utilization of healthcare resources to promote efficiency and cost-effectiveness.

Serve as a patient advocate by supporting informed decision-making and ensuring access to necessary services.

Maintain documentation in accordance with hospital and regulatory standards.

Requirements
Requirements:

Active California Registered Nurse (RN) license.

Minimum of 2 years of recent clinical experience, preferably in case management, utilization review, or discharge planning.

Knowledge of healthcare regulations, payer systems, and community resources.

Strong communication, critical thinking, and organizational skills.

Proficiency in electronic medical records (EMR) system
