Connected Health Care, a leading recruitment and staffing firm specializing in healthcare placements nationwide, is currently seeking a Local Travel Licensed Vocational Nurse (LVN) for an exciting opportunity with one of our valued healthcare partners at a Skilled Nursing Facility in Portola, California. Position Overview
The Local Travel LVN will provide direct nursing care to residents in a Skilled Nursing Facility, working closely with physicians, registered nurses, and interdisciplinary care teams. The ideal candidate will demonstrate strong clinical skills, attention to detail, and a commitment to providing high-quality patient care in a long-term care setting.
Key Responsibilities

  • Provide direct patient care to residents in a skilled nursing and long-term care environment
  • Administer medications, treatments, and injections according to physician orders
  • Monitor and document patient conditions, vital signs, and changes in health status
  • Assist in developing and implementing individualized patient care plans
  • Provide wound care, catheter care, and other clinical treatments as required
  • Communicate effectively with physicians, RNs, CNAs, and other healthcare staff
  • Educate residents and family members on care plans and health management
  • Maintain accurate documentation in accordance with facility policies and state regulations
  • Ensure compliance with infection control standards and patient safety protocols
  • Support daily activities and ensure residents receive compassionate, respectful care
Education and Certification Requirements
  • Active California Licensed Vocational Nurse (LVN) license in good standing
  • Graduate of an accredited Vocational or Practical Nursing program
  • Current Basic Life Support (BLS) certification from the American Heart Association or equivalent
  • Minimum of 1 year of recent LVN experience, preferably in a Skilled Nursing Facility or long-term care setting
  • Strong clinical assessment and patient care skills
  • Ability to work effectively in a fast-paced healthcare environment
  • Excellent communication and teamwork abilities
Compensation and Assignment Details
  • Weekly pay up to $2,295 based on experience
  • 13-week local travel contract with potential for extension
