Position Overview
Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) at a facility, group home, or office-based environment. Compensation $35.00–$38.00 per hour, depending on experience. Shift: 2nd shift 2–10 PM with possible AM coverage; not on weekends.
Work EnvironmentIndoor setting approximately 75% or more of the time, with adequate ventilation and climate control.
Key Responsibilities- Provides direct nursing care and ongoing health monitoring under appropriate clinical supervision, in compliance with BVNPT standards and physician orders.
- Delivers diabetes-related nursing care including blood glucose monitoring, insulin and oral medication administration, symptom recognition, and documentation for individuals with Type 1 and Type 2 Diabetes Mellitus.
- Implements individualized care plans, treatments, and medical protocols as ordered by licensed providers.
- Coordinates medical appointments, specialty care, and follow‑up services for individuals receiving care.
- Provides on‑site clinical support, assessments, and interventions as within LVN scope of practice.
- Collaborates closely with the Program Director, Administrator, and interdisciplinary team to ensure health, safety, and regulatory compliance.
- Educates and supports direct care staff on medical protocols, medication administration, diabetes management practices, and emergency response procedures.
- Communicates appropriately with family members, guardians, healthcare providers, and external stakeholders as needed.
- Represents the organization at medical appointments, interdisciplinary meetings, and external conferences as assigned.
- Maintains knowledge of program regulations and assists with preparation for licensure reviews, audits, and surveys.
- Participates in on‑call rotation to support medical needs outside of regular business hours.
- Completes accurate, timely documentation in accordance with organizational and regulatory standards.
- Performs additional related duties as assigned.
- Current California LVN/LPN license in good standing.
- Demonstrated experience supporting individuals with Type 1 and Type 2 Diabetes preferred.
- Knowledge of medication administration, chronic disease management, and supportive behavioral health environments.
- LVN or LPN CA license; valid driver's license; 1 year experience working with individuals with developmental disabilities and/or mental health issues; experience working in a residential environment preferred.
- Associate Degree.
- 1–3 years of related experience.
