Job Profile Summary

The Nurse Practitioner (NP) acts as part of the clinical operations team and is responsible for providing direct patient care in Chen Med/Jencare medical centers, nursing homes, skilled nursing facilities (SNF) and home settings. Duties include geriatric assessment, medical history, physical exam, diagnosis and treatment, development of the nursing plan of care, health education, physician referrals, case management referrals, follow‑up and clear documentation according to Chen Med standards for quality, service, productivity and teamwork.

The role also involves clinical rounds, conferences and in‑depth documentation through written progress notes and summaries.

Essentials of the Role

The NP must demonstrate the ability to function both independently and in collaboration with other health care professionals, consult with the manager, physician, and medical director to ensure compliance with guidelines. The position may require participation in risk and quality management programs, clinical meetings and other meetings. The NP will adhere to strict departmental goals, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.

ESSENTIAL

JOB DUTIES / RESPONSIBILITIES
  • Independently assess acute and non‑acute clinical problems.
  • Perform and document physical assessments and patient histories, analyze trends in patient conditions, and develop, document and implement a patient management plan in response to the data obtained.
  • Assist in the development of the nursing plan of care, provide appropriate patient/family counseling and education.
  • Plan patient care based on in‑depth knowledge of the specific patient population and or protocol, anticipating and identifying physiological and or psychological problems commonly encountered.
  • Serve as patient advocate, write admission, transfer and discharge orders, and order appropriate laboratory and diagnostic studies.
  • Order appropriate medication and treatments, refer patients for specialty consultation, and document progress notes and summaries.
  • May perform invasive procedures independently upon completion of documented competency.
  • Participate in patient care rounds and conferences, communicate management strategies to the patient care team, and collaborate with multidisciplinary members to ensure successful patient care.
  • Recognize situations requiring immediate attention of a physician and initiate life‑saving procedures when necessary.
  • Use advanced communication skills to problem‑solve complex situations and improve processes and service to patients.

Other responsibilities may include:

  • Collaborate with multidisciplinary team members to analyze and evaluate current health care delivery systems and implement new practice patterns.
  • Participate in outside activities that enhance personal and professional growth.
  • Initiate arrangements and write orders for SNF discharges, complete appropriate paperwork.
  • Work collaboratively with physicians, nurses, PT, social workers, family and key caregivers to transition patients to a lower level of care.
  • Introduce oneself to patients/families, explain the NP role, and facilitate patient/family conferences to review treatment goals and provide education.
  • Facilitate discharge to appropriate level of care and use preferred providers for additional services.
  • Prescribe medication to patients based on State of practice.

Education and Experience Criteria
  • ARNP or similar advanced degree in Nursing required.
  • Current Nurse Practitioner Certification in the State of practice required.
  • Board certification by AANP or ANCC preferred.
  • Current DEA number may be required for Schedule
