Position: RN CASE MANAGER -CALIFORNIA

Responsibilities
  • Coordinate and manage patient care plans for assigned caseload in California.
  • Assess patient needs, eligibility, and required services for appropriate coverage.
  • Monitor and document patient progress, update care plans, and report outcomes.
  • Ensure compliance with state regulations and organizational policies for case management.
  • Collaborate with physicians, therapists, and community resources to secure services.
  • Navigate and resolve barriers to care, including referrals and authorizations.
  • Educate patients and families on disease management, medications, and care expectations.
  • Provide on-site case management support to facilitate timely service delivery.
  • Maintain accurate records for audits, billing, and quality assurance.
  • Utilize and maintain care pipelines and dashboards to forecast patient flow and outcomes.
  • Participate in regular performance reviews and contribute to process improvements.
  • Support growth targets by identifying high‑potential cases and prioritizing them.
Experience/Qualifications
  • RN license valid in California with current nursing practice hours.
  • Experience in case management for medical or managed care settings.
  • Strong knowledge of California state regulations for case management and privacy.
  • Ability to develop and monitor individualized care plans and progress notes.
  • Proven skill in barrier navigation, referrals, and authorization processes.
  • Proficiency with care pipelines, dashboards, and data‑driven forecasting.
  • Excellent collaboration with physicians, therapists, and community resources.
  • Strong documentation, auditing, and billing record‑keeping capabilities.
  • Growth mindset with ability to identify high‑potential cases and contribute to targets.
  • On‑site presence required; ability to travel within California as needed.
