Position:
Licensed Vocational Nurse (LVN) in home health and hospice settings.
Under general supervision, this position provides skilled nursing care to patients in their homes following orders of the attending physician and functions as a member of the interdisciplinary team.
Responsibilities- Reassess nursing needs of patients regularly; assist registered nurse in developing and implementing changes in the plan of care.
- Provide skilled prevention and rehabilitative nursing procedures appropriate to patient care and safety.
- Observe signs and symptoms and report changes to Patient Case Manager.
- Teach, supervise, and counsel patients and family members regarding nursing care needs and related issues.
- Utilize community resources and provide continuity of care through assistance with long‑range planning and follow‑up care.
- Maintain clinical records for all patients, including visit notes, progress notes, and up‑to‑date medication sheets.
- Participate in staff meetings, interdisciplinary conferences, and inservice programs.
- Adhere to administrative and clinical policies and procedures.
- Demonstrate sensitivity and competence in the care of ill and/or dying patients of all ages.
- Post‑High School Vocational or Specialized Training.
- At least 1 year LVN experience.
- California LVN License.
- Current CPR certification.
- Current California driver’s license.
- Current automobile liability insurance.
- Previous home health/hospice experience.
- Additional courses or training in physical therapy or occupational therapy.
- Moderate physical effort (lift/carry up to 25 lbs.).
- Occasional prolonged standing/walking.
- Lifting and moving patients.
- Manual dexterity and mobility.
- Occasional reaching, stooping, bending, kneeling, and crouching.
