Clinical Care Coordinator - Neurosurgery

Mission

The mission of the Coordinator Clinical Care is to identify high risk patients and help coordinate resources to facilitate care, improve health outcomes, and reduce avoidable hospitalizations and readmissions. Ultimately to provide the best quality medical care available, while working toward a patient‑centered medical home and helping ease our patients' way.

• Identify high risk patients through assessment and screening.
• Coordinate and facilitate access to clinical, social, and community resources.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Monitor patient progress and adjust care coordination as necessary to improve outcomes and reduce rehospitalization.

• Graduate of school for medical assistants and certificate., Or
• Coursework/Training:
  Diploma/certificate from the military in Medical Specialist Course.
• 2 years directed patient care at MA level or higher.

Pay range: $24.82 - $37.98. Additional compensation may be available such as shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
