Provide effective, equitable value‑based primary care to adults on Medicare, ensuring patients remain healthy and out of the hospital. Work independently and collaboratively within a multidisciplinary outpatient team, delivering longitudinal care, care coordination, and population health leadership.

• Conduct office visits for routine and acute issues
• Administer annual wellness visits and health risk assessments with holistic, accurate, and specific documentation
• Coordinate care with other providers, specialists, testing facilities, and agencies
• Lead population health initiatives (e.g., ensuring eligible females receive breast‑cancer screening every 2 years)
• Assist the care team with phone triage and outreach
• Educate patients on health conditions, care plans, and treatments
• Participate in Oak Street Health promotional activities
• Conduct home visits as needed
• Other duties as assigned

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• National certification in at least one specialty:
  Family Nurse Practitioner, Adult‑Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, Gerontological Nurse Practitioner
• Active, non‑probationary State Nurse Practitioner license
• Active DEA license
• U.S. work authorization

• Experience in internal medicine, geriatrics, or family medicine primary care
• Knowledge of HCC documentation, ICD‑10 coding, and Health Risk Assessments
