Role SummaryProvide effective, equitable value‑based primary care to adults on Medicare, ensuring patients remain healthy and out of the hospital. Work independently and collaboratively within a multidisciplinary outpatient team, delivering longitudinal care, care coordination, and population health leadership.
Core Responsibilities- Conduct office visits for routine and acute issues
- Administer annual wellness visits and health risk assessments with holistic, accurate, and specific documentation
- Coordinate care with other providers, specialists, testing facilities, and agencies
- Lead population health initiatives (e.g., ensuring eligible females receive breast‑cancer screening every 2 years)
- Assist the care team with phone triage and outreach
- Educate patients on health conditions, care plans, and treatments
- Participate in Oak Street Health promotional activities
- Conduct home visits as needed
- Other duties as assigned
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- National certification in at least one specialty:
Family Nurse Practitioner, Adult‑Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, Gerontological Nurse Practitioner - Active, non‑probationary State Nurse Practitioner license
- Active DEA license
- U.S. work authorization

- Experience in internal medicine, geriatrics, or family medicine primary care
- Knowledge of HCC documentation, ICD‑10 coding, and Health Risk Assessments
