Job Description

Eastern Plumas Health Care is a small, non‑profit, critical access hospital district, providing comprehensive medical services to Plumas County since 1971. We operate a 9‑bed acute care hospital at our main Portola campus, which includes a 24‑hour physician‑staffed emergency room and ambulance service. In addition, four primary care medical clinics and a dental clinic offer residents a full spectrum of health care services in the Graeagle, Loyalton, and Portola communities.

There are also hospital‑based Skilled Nursing Facilities in Portola and Loyalton.

The Certified Nurse Aide (CNA) provides direct and indirect resident care activities under the direction of a RN or LVN. The position is open at the Loyalton campus for full time day/night shifts or part‑time per diem CNA’s. The role includes responsibilities listed below.

Principal Accountabilities
  • Handle and care for patients in a manner conducive to their safety and comfort.
  • Assist in the preparation of and accompany residents during admission, transfer, and discharge procedures.
  • Ensure all resident clothing and personal items are on the inventory sheet and labeled with the resident’s name.
  • Provide morning care, which may include a bed bath, shower or tub bath, oral hygiene, combing hair, back care, dressing residents, changing bed linen, cleaning over the bed table and bedside stand, straightening the room, and other general care as necessary throughout the day.
  • Assist residents to commode, toilet or bedpan when required, empty urinals after each use and rinse them, and assist those residents with toileting and personal hygiene that requires assistance.
  • Provide evening care, including hand/face washing as needed, oral hygiene, perineal care, freshening linen, cleaning over the bed tables, straightening the room, and other general care as needed.
  • Assist with post‑mortem care and transportation of bodies to the mortuary vehicle.
  • Assist RN/LVN in treatment procedures.
  • Provide general nursing care such as positioning residents, lifting and turning residents, utilizing special equipment, assisting with bedpan or commode use, and ambulating residents. Take and record temperature, pulse, respiration, weight, blood pressure, pain level, and intake‑output.
  • Make rounds with oncoming and outgoing shifts; know the whereabouts of assigned residents and ensure the unit is left in good condition.
  • Participate in socialization activities on the unit.
  • Use gait belts, Hoyer lift, and other safety devices appropriately.
  • Check for reddened areas of skin breakdown and report to the L.V.N. or R.N.
  • Ensure residents are dressed properly and assist as necessary. Properly store used clothing in the bedside dresser or on hangers in the closet. Keep residents clean and dry at all times.
  • Check the unit for adequate linen.
  • Treat residents and families with respect and dignity.
  • Review and follow all safety precautions. Follow facility procedures regarding side rails and residents as ordered.
  • Accompany residents to appointments as directed and transport residents to and from various departments as required.
  • Provide reality orientation in daily care.
  • Prepare residents for meals; serve and remove food trays and assist with meals or feed residents as necessary. Distribute drinking water and other nourishment to residents.
  • Answer residents’ call lights, anticipate residents’ needs, and make rounds to assigned residents.
  • Assist residents with handling and care of clothing and other personal property (including dentures, glasses, contact lenses, hearing aids, and prosthetic devices).
  • Record any changes observed in the condition or behavior of residents and unusual incidents when appropriate.
  • Establish and maintain interpersonal relationships with residents, family members, and other facility personnel while ensuring confidentiality of resident information.
  • Attend in‑service education programs as assigned to learn new treatments, procedures, and developmental skills.
  • Attend at least 80% of staff meetings annually.
  • Assist in maintaining a clean hospital environment, including housekeeping duties when housekeeping is not available.
