• Collaborate with the care team to develop, implement, revise, and report the case management plan for a defined patient population
• Act as a liaison among the patient, family, nursing staff, physicians, multidisciplinary team, and healthcare funder to optimize outcomes
• Coordinate a continuum of care from pre-admission through post-discharge for patients admitted to the unit
• Ensure patients and families progress efficiently through hospitalization and participate in discharge planning
• Provide consultation to the healthcare team on patient-specific items related to case management and care coordination
• Contribute to evaluation and treatment considerations for patients with acute psychiatric or behavioral health disorders

• Active California RN license
• Ability to work in a hospital-based psychiatric/behavioral health unit
• Knowledge of case management concepts and care coordination
• Strong communication and collaboration skills
• Ability to work 8-hour day shifts, 40 hours per week
