LVN​/LPN – LTC​/SNF

Job Summary:

Provides skilled nursing care to residents in a skilled nursing facility under the supervision of a Registered Nurse and Director of Nursing. Responsible for administering medications, assessing resident needs, and coordinating care to promote resident well-being and safety.

Responsibilities:

• Assess residents' conditions and determine appropriate nursing interventions.
• Administer medications following the eight (8) medication rights to prevent errors.
• Document changes in residents’ conditions and care provided on progress notes and care plans.
• Perform waived point-of-care testing and interpret results, taking necessary actions.
• Communicate effectively with physicians, staff, residents, and families regarding care and changes.
• Supervise CNAs and other support staff to ensure quality resident care and compliance with policies.

Qualifications:

• Graduate of an accredited LVN program.
• Current California Licensed Vocational Nurse (LVN) license.
• Current BLS certification.
• Ability to read, write, and speak effectively in English; additional languages preferred.
• Basic computer skills with experience or familiarity with Electronic Health Records preferred.
