LPN / LVN - SNF

Profession: Nursing

Shift: Night Shift, Part-time (Wednesday night and Saturday night)

Hours per Shift: 24

Experience: Minimum 2 years of experience

License: Current California Licensed Vocational Nurse

Certifications: Current BLS certification

Must-Have: Skilled nursing experience

Description:

Assesses the resident's condition and nursing needs and prescribes appropriate nursing action.

Ability to adequately assess and reassess pain.

Utilizes appropriate pain management techniques.

Educates the residents and family regarding pain management.

Documents changes in residents' conditions and nursing care on progress notes and care plans.

Ability to perform waived testing (point-of-care testing) per policies and procedures.

Ability to interpret results of waived tests and take appropriate action.

Administers medications and follows the eight medication rights, reducing the potential for errors.

Meets current documentation standards and policies.

Notifies physician of changes in resident's condition and follows through until appropriate action is taken.

Notes and conducts physician's orders in a timely manner.

Gives thorough reports to the oncoming shift by participating in walking rounds and documenting appropriately.

Follows through as needed on information given by shift report, residents, or families regarding concerns.

Implements plan of care for residents based on assessments and goals established by the interdisciplinary care team.

Treats residents and their families with respect and dignity.

Identifies and addresses the psychosocial, cultural, ethnic, and religious/spiritual needs of residents and families.

Supervises other members of the healthcare team as needed to accomplish the plan of care.

Demonstrates competence in technical and manual skills according to scope of practice.

Functions as a resident advocate by protecting residents' rights.

Demonstrates creativity in solving problems and viewing situations in innovative ways.

Interacts with residents, families, co-workers, managers, and other staff in a kind and knowledgeable manner.

Consistently involves residents and families in planning for care, treatment, teaching needs, and discharge plans.

Functions as a leader for staff, delegating in a manner that contributes to excellence.

Participates in orientation of new personnel and assists in performance evaluations when needed.

Follows proper infection control procedures and supervises staff in infection control.

Demonstrates the ability to remain calm in emergencies and/or stressful situations.

Consistently follows facility policies and procedures.

Exhibits genuine interest in and appreciation of adults in later years of life.

Is a motivated, independent individual who can organize workload to ensure completion of all functions.
