Mission Hills, CA
Assisted Hospice is looking for a per diem RN Case Manager for our Mission Hills branch to pick up 10-15 visits per week plus some on-call shifts (nights and weekends are available).
Please note, we pay per visit. See rate of pay below.
Regular visit rates- Admission- $200 per visit + $11 for mileage
- Routine- $70 per visit + $11 for mileage
- Weekday waiting to engage rate- $50 per day
- Weekend waiting to engage rate- $100 per day
- Admission- $200 per visit + $11 for mileage
- Routine- $105 per visit + $11 for mileage

What you will do- Have flexible hours and working close to home (reduce your commute!)
- Complete initial and on-going comprehensive assessments of the patient's needs
- Provide service and /or treatments requiring substantial and specialized nursing skills
- Initiate appropriate preventative and rehabilitative nursing procedures
- Counsel, support and instruct the patient, family and caregiver in meeting nursing and related needs
- Graduate of an approved school of professional nursing
- Current, active license in the state of California Board of Nursing,
- Current CPR, health clearance, TB clearance.
