LVN​/LPN – LTC​/SNF

Job Summary:

Responsible for providing high-quality nursing care to residents in a skilled nursing facility setting. Works under the supervision of the Director of Nursing to assess patient needs and deliver individualized treatments in compliance with facility policies.

Responsibilities:

• Assess residents’ conditions promptly and accurately, reporting abnormal findings to the Charge Nurse and physician.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Administer medications, treatments, and procedures, ensuring accuracy and adherence to protocols.
• Monitor and document patient responses to interventions and progress towards recovery.
• Respond effectively to emergency situations, initiating resuscitation and notifying appropriate personnel.
• Maintain infection control standards and uphold safety protocols to ensure patient and staff safety.

Qualifications:

• Hold an active CA LVN/LPN license with good standing.
• Possess current BLS CPR certification.
• Demonstrate physical stamina to walk, lift, and perform manual nursing skills.
• Exhibit excellent assessment skills, problem-solving abilities, and reliable attendance.
• Communicate effectively in English, both verbally and in writing.
• Show a commitment to continuing education and professional development.
