CNA – LTC​/SNF

Job Summary:

Provides compassionate and individualized nursing assistance to residents in a skilled nursing facility, collaborating with healthcare team members to ensure optimal resident care. Responsible for assessing resident needs, documenting observations, and supporting daily care activities.

Responsibilities:

• Receive and relay detailed reports on residents' conditions to ensure continuity of care.
• Conduct regular rounds to assess residents, monitor new admissions, and respond to changes in health status.
• Document all assessments and resident information accurately according to facility policies.
• Notify nursing staff promptly of any significant changes or concerns related to resident health.
• Assist residents with activities of daily living, ensuring call lights are answered swiftly and compassionately.
• Foster positive communication and working relationships with staff, residents, families, and healthcare providers.

Qualifications:

• Valid California CNA license required.
• Proven experience working in a skilled nursing or long-term care setting.
• Strong communication skills and ability to work effectively within a team.
• Demonstrated ability to assess resident needs and respond appropriately.
• Knowledge of clinical documentation and resident care procedures.
• Flexibility to work weekends, holidays, and float to different units as needed.
