Infection Preventionist (IP) Oak Glen Post Acute Cherry Valley, CA

Oak Glen Post Acute is seeking a dedicated and detail-oriented Infection Preventionist (IP) to lead and coordinate our infection prevention and control program. This role is responsible for ensuring compliance with federal, state, and local infection control regulations while promoting a safe and healthy environment for our residents, visitors, and staff. Responsibilities
  • Develop, implement, and maintain the facility's Infection Prevention and Control Program.
  • Conduct surveillance, tracking, trending, and reporting of infections and communicable diseases.
  • Monitor and investigate infection outbreaks and implement corrective actions.
  • Ensure compliance with CDC, CMS, OSHA, and state infection control regulations.
  • Collaborate with nursing leadership and interdisciplinary teams to reduce healthcare-associated infections.
  • Educate and train staff on infection prevention practices, hand hygiene, PPE usage, and isolation precautions.
  • Maintain infection control documentation and prepare reports for Quality Assurance and Performance Improvement (QAPI) meetings.
  • Conduct audits and provide ongoing monitoring of infection prevention practices throughout the facility.
  • Assist with emergency preparedness and outbreak response planning.
  • Serve as a resource for residents, families, and staff regarding infection prevention initiatives.
Qualifications
  • Current California RN or LVN license preferred.
  • Infection Preventionist experience in a skilled nursing facility or healthcare setting preferred.
  • Strong knowledge of CDC, CMS, and California Department of Public Health regulations.
  • Certification in Infection Prevention and Control (CIC) preferred or willingness to obtain.
  • Excellent organizational, communication, and leadership skills.
  • Ability to analyze data, identify trends, and implement effective interventions.
  • Proficient with electronic medical records and Microsoft Office applications.
