Overview: The Case Manager II will develop, in partnership with clients and multi-disciplinary team partners a trauma informed, client centered, strength-based case management plan, to include: identification of existing strengths and challenges in accordance with treatment plan objectives. Meet with clients on a need assessment bases to assist in removing barriers to meet case treatment goals and linking clients to on and offsite resources.

Assist clients in applying for Crime Victim Compensation. Conduct assertive community outreach. Be responsible for the release of emergency funds and paperwork. Be responsible for contacting TRC representative for approval of funds, when needed. Provide accompaniments to forensic medical evidentiary examinations, court hearings, or psychiatric appointments for minor children and adults. Will complete requests for services when out in the community.

Represent the program and organization at roll-calls with law enforcement and other meetings. Act as an advocate for the client at multidisciplinary team meetings. Maintain accurate documentation, monthly reports, inputting data into the data management systems. Attends staff and meetings, participates in training of staff, volunteers and the community.

Coordinate client transportation to and from sessions. Provide accompaniments to forensic medical evidentiary examinations, court hearings, or psychiatric appointments for minor children and adults. Will complete requests for services when out in the community.

Qualifications: Bachelor's or Master's Degree in a related field preferred. Bilingual English/Spanish preferred, but not required. Has a minimum of two years of relevant experience working with adults, families and/or children who have experienced one or more of the following: physical abuse, sexual abuse, sexual exploitation, and/or emotional abuse; neglect, including Parental chemical substance abuse; domestic violence and/or witness to violence in the home, child abduction, exposure to crime/violence in the community, victim of other criminal activity and or bullying per California Education Code
48900(r). Must be a highly organized individual able to multi-task, track and document case management of clients. Computer competency, including Word, Excel, Outlook and PowerPoint.

Knowledge, Specialized Skills, and Abilities: Must demonstrate the ability to use communication skills that successfully convey and obtain accurate and complete information in a respectful and caring manner. Have a basic knowledge of body language and non-verbal communication. Understand and demonstrate active listening skills. Must be a critical thinker, reasoning, organizing and analyzing information so that problems are accurately understood and solutions are congruent with client's capacities and outcome orientated.

Capacity to negotiate and collaboration to insure the client's successful receipt of services. Demonstrate the skill of advocating for the needs and rights of people. Must show a passion and sensitivity for trauma work. Demonstrate experience to maintain cooperative working relationships with others; be team oriented; coordinate/collaborate with professionals from multiple agencies whilst maintaining appropriate boundaries and client confidentiality.
