About the roleThe April Parker Foundation is seeking compassionate, detail‑oriented Regional Care Managers to deliver In-Person Enhanced Care Management (ECM) and Community Supports (CS) services to Medi‑Cal members with complex medical, behavioral, and social needs.
This field‑based position is ideal for professionals who value flexibility, autonomy, and meaningful impact. Regional Care Managers are assigned a territory and engage clients in person to address their physical health, behavioral health, and social determinants such as housing, food access, and transportation.
In‑person engagement is required, with a minimum of one in‑person visit per member per month within your assigned region.
What you'll do- Conduct outreach, intake, and comprehensive assessments for ECM and CS members
- Develop and implement individualized care plans
- Coordinate medical, behavioral health, and social services, including housing navigation, tenancy support, food access, and transportation resources
- Provide in‑person, telephonic, and virtual services based on member needs
- Maintain accurate, timely documentation in APF systems
- Collaborate with health plans, community partners, and internal multidisciplinary teams
- Ensure compliance with program requirements
- Support members in achieving long‑term stability and self‑sufficiency
We offer a transparent, performance‑driven compensation model that rewards active engagement and in‑person service delivery.
Base Pay: $25.00 per hour for all hours worked. (Approx. $4,000 per month based on 40 hrs/week)
In-Person Performance Incentive Pay: $40.00 per member per month for each member who receives a qualified in‑person ECM or CS service. Care Managers who consistently serve a caseload of 40–60 active members can earn up to $6,400 per month.
Reimbursements & Stipends: Mileage at IRS rate and approved work‑related expenses.
ScheduleMonday–Friday, with occasional weekend and evening availability as required. The majority of hours are field‑based; this is not a home‑office role.
QualificationsRequired:
- Minimum 2 years of experience in case management, care coordination, or social services
- Knowledge of Medi‑Cal / CalAIM programs, community resources, and social determinants of health
- Strong documentation and organizational skills
- Excellent communication and relationship‑building skills
- Access to reliable transportation
Preferred:
- Bachelor’s degree in a related field
- Fluent Spanish speaker
- Community Health Worker (CHW) certification
- California licensure (LCSW, LMFT, LPCC, RN, or similar)
