POSITION SUMMARY:

Under the direction of the BHBH Program Manager, the BHBH Case Manager provides comprehensive, trauma-informed, and culturally responsive case management services to individuals and families experiencing homelessness or at risk of homelessness.

This position is responsible for housing stabilization, supportive service coordination, rental assistance oversight (short-term and mid-term), life skills development, and long-term self-sufficiency planning. The Case Manager works directly with clients to remove barriers to stable housing, increase income, improve behavioral health outcomes, and promote independence.

The ideal candidate has strong experience working with homeless populations, behavioral health challenges, and individuals with complex needs, and is skilled in delivering life skills workshops and client-centered case planning.

JOB DUTIES AND RESPONSIBILITIES:

* Conduct comprehensive assessments including housing, behavioral health, employment, and financial stability needs

Develop individualized Housing Stabilization Plans with measurable goals

* Coordinate short-term and mid-term rental assistance in alignment with grant guidelines

* Provide intensive case management focused on long-term housing retention and self-sufficiency

* Facilitate life skills workshops (budgeting, tenant rights, employment readiness, financial literacy, healthy relationships, relapse prevention, etc.)

* Connect clients to employment services, workforce development, and income supports

* Assist clients in obtaining identification documents, benefits enrollment, and community resources

* Provide housing navigation including landlord engagement and lease support

* Monitor client progress and adjust service plans as needed

* Maintain accurate and timely documentation in compliance with grant and Tribal requirements

* Collaborate with behavioral health providers, tribal departments, housing providers, and community agencies

* Conduct home visits and field-based case management when appropriate

* Support culturally grounded healing approaches and incorporate Tribal values into service delivery

* Participate in case staffing meetings and multidisciplinary coordination.

* Assist with community outreach and engagement activities

* Support data collection and grant reporting requirements

* Participate in program development and quality improvement efforts

* Attend relevant trainings related to homelessness, behavioral health, and housing stability

* Perform other duties as assigned

KNOWLEDGE, SKILLS, CERTIFICATIONS AND ABILITIES:

* Strong knowledge of homelessness systems, housing stabilization, and rental assistance programs

* Experience working with individuals experiencing homelessness, substance use disorders, and mental health challenges

* Ability to facilitate group workshops and life skills training

* Knowledge of trauma-informed care and strengths-based approaches

* Familiarity with coordinated entry systems and housing navigation

* Understanding of culturally responsive services, particularly within Native communities

* Ability to de-escalate crises and provide stabilization support

* Strong documentation and organizational skills

* Ability to work independently and collaboratively

* Commitment to ethical practice and confidentiality

MINIMUM QUALIFICATIONS:

* Bachelor's degree in social work, Human Services, Psychology, Sociology, or related field

* OR equivalent combination of education and experience

* Minimum two (2) years of experience working with homeless populations, housing programs, behavioral health, or intensive case management

* Experience facilitating workshops or group-based life skills education preferred

* Knowledge of housing-first and harm-reduction models preferred

* Valid California Driver's License and clean driving record
