Kaiser Permanente seeks a Case Manager Continuing Care Social Work professional in our Healthcare & Medical Services department. In this role, you will coordinate post-acute and ongoing care for diverse patient populations, conduct psychosocial assessments, and develop person-centered care plans. You will collaborate with physicians, nurses, and community agencies to ensure safe transitions of care, connect patients with appropriate resources, and advocate for their needs.
You'll also track outcomes, maintain accurate documentation in the EMR, and contribute to quality improvement initiatives that enhance continuity of care and patient well-being.
