PROGRAM AND

LOCATION:

VOA-NCNN, a Certified Great Place to Work, is recruiting for a full-time Case Manager II for the Family Program at our Mather Community Campus. Full-time employees (working 30 or more hours a week) are eligible for a benefits package that includes paid holidays, vacation, sick pay, health, dental, vision, life, and short-term disability insurance coverage.

Responsibilities

POSITION SUMMARY:

Under direction of the Program Manager and with independent professional responsibility, the Case Manager II performs case management with individual program participants and families assisting Case Manager I in the Mather Family Program. The Case Manager II functions at a highly skilled level in such areas as assessment, individual goal planning, group facilitation, community resource development, file monitoring and review, and reviews agency reports and develops summaries.

Working alongside the Housing Specialist and Employment Specialist, Case Manager II will help develop successful outcomes for clients. This position is committed to improving the quality of services provided by seeking input from participants, employees and stakeholders to ensure service delivery and accessibility is equitable to all persons served.

OBJECTIVES/

ACTIVITIES:

Responsible for performing daily work requirements to achieve established objectives of the department.

  • Assist Case Manager I, Housing Specialist, and Employment Specialist with client barriers.
  • Perform overall assessment of program participant’s needs and develop an individualized service plan with short- and long-term goals.
  • Review case plans weekly to adjust goals where needed or appropriate.
  • Educates program participants on issues surrounding living skills, parenting, financial capability, workforce development, health and hygiene, housing, and drug and alcohol abuse.
  • Develops and maintains comprehensive lists of community resources for use by program participants; refers program participants accordingly.
  • Documents all contact with program participants; prepares all screenings, assessments, service plans, evaluations, and reporting records.
  • Provides crisis intervention, referrals and collaborative consultation with any service providers working with the participant.
  • Maintains and updates all participant files to ensure compliance.
  • Acts as a peer review committee and will conduct monthly file review of participant files.
  • Attends and conducts meetings and training, as directed.
  • Reviews Case Manager I client case notes/services on HMIS, to keep in compliance.
  • Assists participants with mediation, critical thinking, and problem solving.
  • Supports Case Manager I with participants and caseload when needed.
  • Supports effective communication between Case Managers, Employment Specialists, and Housing Specialists.
  • Establish and maintain positive relations with the general public, funding agency, participants and other staff.
  • Ensures compliance with all VOA policies, CARF standards, and safety and security regulations.
  • Performs other duties as assigned.
